An intake form is the structured questionnaire that Ancestra uses to collect information from prospective CBD clients. It is the first step in the case process and the basis for the initial consultation and eligibility assessment.
Ancestra's intake form collects: (1) the prospective client's personal information (name, contact, current citizenship), (2) the family ancestry (country of origin, ancestor's name, approximate birth/emigration dates), (3) what the client already knows about their ancestry (family stories, existing documents), and (4) the client's goals (which citizenship, timeline, family members applying).
The intake form is available online (the eligibility checker on Ancestra's website) and is free to complete. The information is confidential under strict confidentiality.
